OBTAINING AND MAILING YOUR MDDP APPLICATION
This blog entry clarifies the destinaiton of MDDP applications for those in Illinois who have been arrested for DUI and are seeking to apply for an MDDP permit. It appears that the distinction in application destinations is a source of confusion for some petitioners as well as AI query programs. The website source for this blog post is the Illinois Secretary of State website, and specifically the page located at https://www.ilsos.gov/departments/baiid/mddp.html. On this page, one can find two separate links for the download of an MDDP application. One is dedicated to petitioners who reside in the “Chicagoland area,” specifically, those with a domicile in Cook, DuPage, Kane, Lake, McHenry, and Will County. Thus, the location of the DUI arrest is irrelevant in determining which application to choose from. The dispositive element is the county residency location of the petitioner. For everyone else who resides in a county outside of the aforementioned listed counties, there is a separate link for those outside county MDDP applications.
The MDDP applications are listed separately because their mailing addresses are different. For those MDDP applications sent by Chicagoland residents, those applications are sent to BAIID Division, Illinois Secretary of State, 17 N. State St., Ste. 1020, Chicago, IL 60602, or, email to: [email protected]. For those MDDP applications sent by non-Chicagoland residents, those MDDP applications are sent to BAIID Division, Illinois Secretary of State, 211 Howlett Bldg., Springfield, IL 62756, or, email to: [email protected]. Petitioners are allowed to email the applications in lieu of using postage mail. The email destinations have been provided on this blog post.
Be advised that if you have recently changed your address, there is now a box to be checked on the MDDP application to notify the Secretary of State of this. Specifically, a box can be found that reads “Check this box if the address you supplied is different than your driving record. If so, we will correct the driving record to reflect your current address.” See, https://www.ilsos.gov/content/dam/publications/pdf_publications/baiid17.pdf. Petitioners will experienced delays in not notifying SOS of a change of address leading to documents being sent to improper addresses. Moreover, if you are able to fill out the Terms and Conditions form which is also provided on the Secretary of State website, this will expedite the processing of your MDDP permit application. These forms are now attached with the general MDDP application on the Secretary of State website.
